If you are conducting a PAS assessment and would like to gather background information about the program, you may use this form. If you are submitting PAS assessments for certification or to generate a computerized PAS Report, you must complete this form for each center.
Complete and submit this form with each of your PAS Certification or Recertification Assessments. The Cover Sheet will help to ensure you submit everything you need for the certification process and contains valuable information on assessor protocol for materials.
Participants who attend PAS/BAS Reliability Training and are reliable with the national anchors are eligible to become certified. If certification is achieved they will be listed on the McCormick Center’s Web site as a certified PAS/BAS assessor. An Item 23, Lead Teacher page should be completed for each classroom in a center. Only one person should be designated as the Lead Teacher per classroom. The definition of the Lead Teacher can be found on pages 3, 54, and 63 in the PAS.
Use these labels in your PAS book to determine the rating for Item 24 indicator 1.2.
An Item 25, Assistant Teacher/Aide page should be completed for each teaching staff member in a classroom that meets the definition of a Assistant Teacher/Aide found on pages 3, 58, and 63 in the PAS.
Duplicate this form if you have more than five classrooms to determine the average PAS scores for Items 23, 24, and 25.

These labels can be used by directors who are organizing their documentation by item. Print off the file folder labels and place them on a file folder tab. Then place any supporting documentation related to the item in the appropriate file folder.
