Coordinating Care among Service Providers: It’s for the kids

Coordination of care and communication between service providers and the medical home is essential to effectively meeting the complex needs of children and families. Participants will learn the value of documentation tools to standardize the referral and care coordination process and then strategize how to adapt these tools as elements of an organizational protocol for care coordination.
Developing a Trauma-Informed Organizational Protocol
Friday | 2-4 p.m.
Providing trauma-informed care requires a multi-pronged approach inclusive of public education and awareness, prevention and early identification, and trauma-specific assessment and treatment. This workshop will enable participants to evaluate their own organizations and strategize ways to develop a trauma-informed organizational protocol, utilizing 6 key principles and 10 implementation domains.
